Here are important documents for camp. Below, you’ll find links for an info packet, a fillable health history, and a parental release to print, sign in ink, and return to The Center.
Please do try to download, “save as,” fill in, save again, and return via email the health history. If your computer situation does not allow this, please call Mark Walker at 708.361.3650. He’s glad to take the info over the phone, and the process should take just five minutes or so.
